Print Player Registration Form

HOW TO PRINT A PLAYER REGISTRATION FORM

Go to the Santa Monica AYSO Registration page:  https://ayso20.d4sportsreg.com/ 

  1. Log-in as an “Existing Member”(The e-mail address you originally registered with and password will be required)
  2. Click on your Player's Name in the Sub-Account(s) box   (Lower middle of this page)
  3. Click on the FORMS button (on the left side of the resulting screen)
  4. Click on the Player Registration Form next to AYSO Fall (Current Year)  (on the right hand side of the screen)to download the Registration Form in PDF format.
  5. Print the form
  6. Check the form for accuracy
  7. If necessary update your contact information (by using the same Santa Monica AYSO Registration page listed above).
  8. Repeat above steps if you make any changes.
  9. Sign the form and give it to your Coach or Team Manager
